Universal-Select Changes:

Due to budget constraints during the current state biennium, the immunization section will be transitioning from a Universal vaccine supply policy to Universal-Select effective July 1, 2011.

Effective July 1, 2011, providers may continue to immunize non-VFC-eligible children (i.e. children with health insurance that covers immunizations) using publicly-provided vaccines with the exception  of the following vaccines:

  1. Hepatitis A

  2. Human papillomavirus (HPV)

  3. Influenza

  4. Meningococcal conjugate

Providers may offer these vaccines to non-VFC-eligible children using private stock vaccines.  Providers may charge the parents/guardians fro the cost of the privately-purchased vaccines as well as vaccine administration fees.

Do you want to fill out your child's vaccination form before arriving at Platte County Public Health?  If you cannot be present with your child when they get their immunizations, you can print this form, fill it out, sign it, and have your child bring it in with them.  We can no longer take approval by phone from parents.  We need to have a signed vaccination form before giving shots.

As required by the Health Insurance Portability and Accountability Act of 1996 (HIPAA), clients have a right to request their health information.  This includes your child's immunization record.  In order to honor this request, we will need to have an "Access to Records Request" form filled out. Your child's immunization record may not be immediately available to you and may take a couple days for us to process.  You can fill out this form in our office or print the form in advance, fill it out, and sign it before you come in.

What you need to know before the vaccine is given:

  • Are you sick today or have an increased temperature (greater than 100.4)?

  • Do you have any allergies to food or medications?                                                    

  • Have you ever had a serious reaction to a vaccine?

  • Do you have cancer, leukemia, AIDS, or any other immune system disorder?

  • Do you take cortisone, prednisone, other steroids, anticancer drugs, or x-ray treatments?

  • Have you received a blood transfusion in the last year?

  • Are you pregnant or planning to become pregnant in the next month?

  • Have you received any vaccines in the past 4 weeks?

These questions will be asked when you come to Platte County Public Health for a vaccine.  Make sure you bring your immunization card with you.

If your child's records are in the Wyoming state registry, we can provide a copy upon request.

We request a $10.00 donation for each injection to help offset cost of supplies.  We will bill Medicaid for this fee, but you have to bring in proof of Medicaid status.  If your insurance company reimburses for childhood vaccines, you will be provided with a 1500 Form to submit.

FYI:

We do not give shots after 4:30 p.m.  The individual receiving a vaccination must wait 15 minutes afterwards.  We monitor for possible reactions to the vaccine.  We don't want anything to happen to someone after our office closes.  If you come in by 4:30, we still have time to monitor you before we close our office for the day (closing time is 5:00 p.m.).  We do this for your safety, and we apologize for any inconvenience regarding this policy.
